Director of Care Coordination

Job Title: Director of Care Coordination   |   Department: Care Coordination

Description:  The purpose of this job is to lead our team in Care Coordination, which focuses on ensuring the most appropriate use of medical services and resources for our hospitalized patients.

Education:  Possess a Social Work degree.

Preferred: Bachelor’s preferred, master’s degree a plus. 

Licensure/Certification/Registration:   **Must possess a valid New York State Social Work licensure. 

Experience:  Must have experience in case management and/or utilization review in acute care, home care or insurance setting. Prior experience with Meditech E.H.R. information systems is a plus. 

Knowledge:   Familiar with commonly accepted medical necessity guideline resources and familiar with managed care industry UR program requirements. 

Shift: 8:00 AM – 4:30 PM   |    Status: full-time (40 hours) – This position does require working one Saturday per month.

Salary: $75,000 – $110,000 / YEAR
