Health Coach

The Family Counseling Center of Fulton County, Inc. is dedicated to improving the lives of individuals and families through comprehensive support, focusing on both physical and mental health. We’re seeking a passionate and dedicated Health Coach to join our team.

Job Summary:
As a Health Coach, you'll empower clients to improve their health and well-being by providing one-on-one coaching, promoting lifestyle changes, and integrating health services with behavioral health care. You'll collaborate with a multidisciplinary team to help clients manage chronic conditions, increase physical activity, and support mental health recovery.

Key Responsibilities:

• Provide personalized coaching to help clients set and achieve health goals.
• Educate clients on chronic disease management, nutrition, and stress management.
• Collaborate with behavioral health providers to address both physical and mental health needs.
• Conduct health assessments, set goals, and track client progress.
• Coordinate care and connect clients to necessary health services.
• Maintain accurate records and contribute to program development and quality improvement.
• Participate in staff development and agency training activities.

Qualifications:

• Bachelor’s degree in Health Education, Public Health, Nursing, or related field.
• 2+ years of experience in health coaching or wellness.
• Certification in Health Coaching (preferred but not required)
• Experience in behavioral health or with mental health/substance use disorders is a plus.
• Knowledge of chronic disease management, health behavior change, and wellness strategies.
• Strong communication skills and ability to engage diverse populations.
• Ability to work independently and in a team environment.
• Knowledge of trauma-informed care and recovery principles.

Job Type: Full-time

Pay: $24.00 per hour

Expected hours: 40 per week
