Social Worker

TITLE: SOCIAL WORKER    DEPARTMENT:  SOCIAL WORK

JOB PURPOSE:  To provide a full range of medical, social and case management services to meet the bio-psycho-social, financial, emotional and cultural needs of patients, outpatients and their families.  Taking into account the individual patient’s special physical or age related needs.  To assist with patient/family and other hospital disciplines access to necessary services that extend to the continuum of care beyond the hospital stay.

EDUCATION:  Possess a bachelor’s degree in related field. 

LICENSURE/CERTIFICATION/REGISTRATION:   None

EXPERIENCE:  Three (3) year’s in Social Services.

PREFERRED:  Three (3) year’s in Social Services in a healthcare Industry.

SHIFT: 8:00AM – 4:30PM (Sunday – Thursday)        STATUS: 40 HOURS

RATE OF PAY: $25.14 – $36.45 / HOUR
