Care Navigator

The Family Counseling Center of Fulton County (TFCC) is looking for a Care Navigator to support client engagement, streamline access to care, and ensure seamless service coordination. What You’ll Do:

• Guide clients through our services and match them with the care that best meets their needs
• Collaborate with clinical and intake teams to assess client needs and determine eligibility
• Coordinate internal and external referrals, ensuring timely and appropriate service connections
• Educate clients and families on available resources and assist with appointment scheduling
• Maintain accurate records in the electronic health record (EHR) system
• Develop partnerships with community organizations and follow up to ensure continuity of care

Qualifications:

• Bachelor’s degree in Social Work, Human Services, Public Health, or related field (Master’s preferred)
• 1–2 years of experience in healthcare, behavioral health, or social services
• Knowledge of NYS Medicaid, Health Home programs, and community resources
• Exceptional interpersonal and communication skills
• Commitment to person-centered, trauma-informed, and recovery-oriented care
• Strong organizational, problem-solving, and documentation skills
• Ability to work independently and collaboratively within a team
• Must pass a Medicaid exclusion screening and background check

Job Type: Full-time

Pay: $24.00 per hour
