What does a Habilitation Coordinator do?

A Habilitation Coordinator plays a key role in working with the people we support and their staff, to create an individualized support plan by assessing the person's interest and skills along with their needs. The Habilitation Coordinator will monitor the progress of each plan and ensure plans are being implemented properly.

Qualifications:

  • Bachelor's degree or associate degree in human services or related field is required.
  • One year experience working with people with developmental disabilities is preferred.
  • Valid NYS Driver's License with 2 years of experience required
